Complaints substantiated: no criminal/disciplinary issues

The Police Ombudsman may uphold a complaint but decide that no individual officer should be prosecuted or disciplined. This may be because she deems that the issue was caused by a lack of training or appropriate guidance, in which case it would be inappropriate to hold an officer individually responsible.

Alternatively, there may be evidence of misconduct, but it may be impossible to identify the individual officer responsible.
